Treatment of Idiopathic Varicoceles by Transfemoral Testicular Vein Occlusion

Percutaneous transfemoral occlusion of the testicular vein is a new alternative in the treatment of idiopathic varicocele. Embolization with the Gianturco coil was done on 27 patients. The occlusion was performed immediately after diagnostic phlebography. The results of followup for 1 year are available for 18 patients. In 1 patient the coil had to be removed because of persistent pain, although cause and effect could not be proved. Two patients had a recurrence. The urographic findings were normal 1 year after embolization. No dislocation of the foreign body was observed. Two patients reported that their wives were pregnant.

The role of Doppler ultrasonography in determining the proper surgical approach to the management of varicocele in children and adolescents

In a study from Italy, colour-Doppler ultrasonography was a reliable diagnostic tool in the preoperative assessment of patients with varicocele. The authors also found that it helped to distinguish those who could be treated laparoscopically from those who should be treated by microsurgical subinguinal ligature. OBJECTIVE To investigate whether colour Doppler ultrasonography (CDUS) is a reliable diagnostic tool for selecting patients with varicocele to undergo either laparoscopy or open microsurgical subinguinal ligation. Between 1973 and 1991 we performed 160 percutaneous angiograms (130 arteriograms, 30 phlebograms) in children and infants; 12 patients were less than one year and 52 less than ten years old. 44 of the examinations were done by a DSA technique. The examinations were carried out under general anaesthesia except in 8 cases. In 50.7% an arteriogram was carried out for the investigation of a suspected or known tumour, in 9.3% an arteriogram was required following trauma. The most common phlebographic examination was for the demonstration of the spermatic vein; in 27 patients this was done for cryptorchidism or a varicocele.
